The St. Lawrence Parish Youth Faith Formation (CCD) program is for school children, grades pre-kindergarten through eighth grade, who attend public or non-Catholic private schools.  Children who are home-schooled and those who attend Villa Madonna, Academy of the Holy Names or Morning Star School and are going to prepare for First Communion and/or Confirmation may also register for the sacraments at this time.

The Family Handbook and registration forms are available below. Download both forms and complete all sections using BLOCK print. 

Please return the forms to the Parish Office Monday through Friday 9:00 a.m. to 5:00 p.m.  If you are completing the Emergency Medical Release Form, you will need to have it notarized before you submit it to us.
